Use form SSP2 to record details of an employee’s Statutory Sick Pay (SSP) if you’re an employer.

How to complete the form

You need to:

  1. Download and save the form on your computer.
  2. Open it using the latest free version of Adobe Reader.
  3. Complete it on-screen.

Statutory Sick Pay: no employee entitlement (SSP1)
Use this form if a sick employee isn’t entitled to SSP or if your payments of SSP to an employee end. Your employee will need it to make a claim for Incapacity Benefit.

  1. Statutory Sick Pay: record sheet (SSP2) has been updated to reflect changes brought in by the Employment Rights Act 2025, to remove the waiting period before SSP can be made. The form can now be filled in onscreen. A Welsh translation has been added.
